Family Care Community Partnership
Family Care Community Partnership (FCCP) is a collaboration between The Providence Center, Family Service of Rhode Island, Comprehensive Community Action Program, Children's Friend and Service and Tides Family Services that brings together community-based service providers, family members and friends to build a stronger, brighter future for children and families at risk for abuse and neglect and children and families who have serious behavioral health needs.
Eligibility:
- Children and families at risk of involvement with the Rhode Island Department of Children, Youth and Families
- Families with children ages birth to 18 years old who have serious emotional, behavioral and/or mental health challenges
- Families with children/youths enrolled in Positive Educational Partnership schools or participating in early childhood settings
- Families with children/youths who are transitioning from the Rhode Island Training School back into the community
Services are provided in the home and community and include:
- Assessment
- Linkages to community programs and resources
- Individualized treatment plan
