Providence Community Health Center at North Main

Providence Community Health Centers at North Main, a collaboration between The Providence Center and Providence Community Health Centers, is a patient-centered medical home that provides total health care for adults experiencing mental health and substance use problems. 

Patients receive coordinated physical, mental health and substance abuse care under one roof with access to community supports that promote a healthy lifestyle.

Services include:

  • Primary care services by a skilled, compassionate primary care physician and staff
  • Primary care support from a Providence Center behavioral health liaison who works with the primary care physician to address each patient’s unique mental health and substance use needs
  • Shared medical and mental health records for efficient communication
  • Access to The Providence Center’s behavioral health and wellness program

Accessing Care

To take advantage of the services at Providence Community Health Centers at North Main, you must choose A. Sattar N. Memon M.D., as your primary care physician.

Dr. Memon attended medical school at the Baroda Medical College in India and completed his Internal Medicine Residency at Catholic Medical Center in Queens, New York. He did a fellowship in Immunology/Oncology at Brown University-Memorial Hospital. He is board certified in Internal Medicine and is a Clinical Associate Professor of Medicine at Brown University. Dr. Memon has written over 20 articles in the area of general medicine and immunology/oncology, which have been published in various journals.
