Continuing Treatment

Happy senior womanContinuing Treatment offer services that help individuals with severe and persistent mental illnesses to avoid hospitalization and live as productively as possible in the community and among family and friends.   

 

In addition to providing traditional clinical services aimed at symptom management, specialized staff members work with individuals to:

  • Locate safe, affordable housing
  • Develop independent living skills
  • Secure and maintain meaningful employment
  • Identify higher education opportunities
  • Participate in community, therapeutic, social and recreational activities

Clients’ clinical, rehabilitative and recovery needs are addressed in their homes, schools and in the community.  Services include:

  • Outreach evaluation
  • Medication management
  • Crisis intervention
  • ADL training
  • Social skills training
  • Coordination with health services and criminal justice systems
  • Community integration
  • Housing
  • Addiction treatment services
  • Vocational services
  • Coordination of services with other community agencies
  • Involvement with client's support network
  • Client advocacy
