You will be asked:

  • To provide your name, address, telephone number;
  • The name of your insurance provider;
  • To describe the nature of your problem;
  • Whether your problem requires emergency care;
  • What, if any, type of treatment you have received in the past;
  • If you are taking any medications.

To ensure that you receive the most appropriate and best course of care, you will also be asked whether you have had any history of substance abuse.

We will then schedule an appointment for you with a mental health professional. We will not schedule you for an appointment more than two weeks in advance, so if there is no availability, or if we cannot provide you with the services you need, we will refer you to an alternative provider.

To make an appointment, call 401-276-4020,
Monday - Friday, 8:30 a.m. to 4:30 p.m.
