520 HOPE STREET, PROVIDENCE

Traveling on 95 North:
Take exit 24 (Branch Avenue). Turn right off the exit. Continue to traffic light, go straight up the hill, Kentucky Fried Chicken will be on the right. Continue straight through first major intersection. The Providence Center parking lot is at the top of the hill on the right.

Traveling on 95 South:
Take exit 24 (Branch Avenue). Turn left off the exit. Continue to traffic light, go straight up the hill, Kentucky Fried Chicken will be on the right. Continue straight through first major intersection. The Providence Center parking lot is at the top of the hill on the right.

530 NORTH MAIN STREET STREET, PROVIDENCE

Traveling on 95 North:
Take the State House exit. At end of ramp, take left onto Orms Street. At first light, take a left onto Charles. Take your first right onto Hawes Street. The Providence Center is the only building on the street. (Parking is on the right, before the building).

Traveling on 95 South:
Take exit 24 (Branch Avenue). Turn left off the exit. Continue to traffic light. Take a right onto North Main Street. Go through 3 lights. On the right is the entry to a parking lot (sign is on the building and marks lot entry). Go down at least 1 level before parking.

#56 MATHIAS BUILDING, ELEANOR SLATER HOSPITAL, CRANSTON CAMPUS, 111 HOWARD AVENUE, CRANSTON

Traveling on 95 South:
Take Route 95 South. Take Exit 14 onto Route 37 West. Take first off ramp (0.4 miles) “Pontiac Avenue, Howard Center.” Turn left at light on Pontiac Avenue and proceed to Howard Avenue (0.8 miles). Turn right onto Howard Avenue. Continue for 0.2 miles to stop sign. Take a right and travel 0.1 miles to a sign on left: MANUAL MATHIAS BUILDING. Take left (and stay toward the right) and travel up hill. At top of hill through parking area for staff and visitors, TPC entrance is after the MHRH Laboratory (last entrance door on right).

Traveling on 95 North:

1070 MAIN STREET, PAWTUCKET

From 95 North:
Take the North Main Street exit (25A). Follow road to the first traffic light (Frost Street and North Main Street) and take a left onto North Main Street. Shortly after passing Shaws on the left, the road will split. Bear to the left. After the first traffic light, park in the gated parking lot on the left across from our red brick building, 1070 Main Street.

From 95 South:
Take the North Main Street exit. Take a left off the exit. Follow the road through the lights and bearing right (the road becomes one-way). Take a left onto Frost Street and proceed to North Main Street (follow above directions).

2198 WALLUM LAKE ROAD, BURRILLVILLE

From 95 North or South:
Take the Rte. 6 exit. Stay in the right lane and take a right hand exit onto Rte. 6 West/RI 195. Exit to Interstate 295 North. Travel on 295 North to te. 44 West (Greenville) exit. Travel West to Chepachet and take Rte. 100 North. Stay on Rte. 100 all the way to Zambarano Hospital (you will see signs to Zambarano). When approaching the hospital grounds, take your third left up the hill and bear right. You will see the Talbot Long-Term Care facility on your left. There is a basketball hoop and gazebo in front of the building. (The building says "Nurses Home")
