The Providence Center is dedicated to helping each client achieve optimal physical and mental health. And though one of the provisions of the Affordable Care Act, Medicaid clients with severe mental illness have access to Health Homes, a program that coordinates all medical services for clients with chronic illnesses. In 2012, Rhode Island became one of the first two states to implement the program.
For a patient with many primary medical and mental health needs that might be challenging to manage, Health Homes ensures consistent coordination of all aspects of care, including psychiatry, primary care, specialty care, medication, hospital discharge and wellness needs. Health Homes are based on the concept of person-centered care, a tenet of The Providence Center’s approach to care.
Participation in the Health Homes program is for Medicaid-insured clients only and who have severe and persistent mental illness.
