Integrated Community Treatment Teams

Formally known as the Health Home Teams program, The Providence Center is dedicated to helping each client achieve optimal physical and mental health. Through one of the provisions of the Affordable Care Act, Medicaid clients with severe mental illness have access to Integrated Community Treatment Teams, a program that coordinates all medical services for clients with chronic illnesses. In 2012, Rhode Island became one of the first two states to implement this program.

For a patient with many primary medical and mental health needs that might be challenging to manage, this program ensures consistent coordination of all aspects of care, including psychiatry, primary care, specialty care, medication, hospital discharge, and wellness needs. Integrated Community Treatment Teams are based on the concept of person-centered care, a tenet of The Providence Center’s approach to care.

The Providence Center develops a treatment plan for each client that addresses all of their health needs, including:
  • Primary care
  • Mental health
  • Substance use treatment
  • Chronic disease management
  • Non-clinical services such as connections to community support and identifying family support

Participation in the program is for Medicaid-insured clients only and who have severe and persistent mental illness.
